Employment Application

Physical Therapist

Outstanding opportunity to become a member of a comprehensive interdisciplinary team in a progressive organization working with young children and adults with neurological impairments, TBIs or developmental disabilities at the Canandaigua, NY site.

Full time vacancy (37.5 hours per week) – School Year Schedule with competitive salary and comprehensive benefit package.

Therapist duties include, but are not limited to:

  1. Upholds New York State Education Law/Practice Act as relating to the practice of physical therapy.
  2. Evaluates individuals to determine the need for physical therapy services.
  3. Ensures valid prescriptions are written for all individuals on caseload.
  4. Carries out treatment plans as developed after evaluation, in various school and community settings and Article 28 outpatient clinic
  5. Consults with teacher, as well as other members of the interdisciplinary team as appropriate to facilitate delivery of coordinated quality services to individuals.
  6. Works with parents as integral members of the interdisciplinary team at school as well as in the home setting.
  7. Communicates with physicians, orthopedists, and other medical personnel in order to facilitate evaluation and treatment of individuals.

Qualified candidates require:

  1. Bachelor’s degree in Applied Science.
  2. License or eligible for license as a Physical Therapist in New York State.
  3. Current New York State registration as a Physical Therapist.
  4. Valid New York State driver’s license.

Residential Assistant

Positions located in Gorham, NY in a residential setting for young adults and teens with disabilities. Successful applicants must have a high school diploma and valid NYS driver's license. Must be able to obtain and maintain Medical Certification. Experience with special needs population preferred.

Full and part time positions working evening shift, overnight shift or weekends as well as substitute opportunities.

Residential assistants provide oversight to ensure safety and supervision of residents. Provide assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to live more independently.

Accompanies residents to off-site locations/activities helping them to access and utilize community resources. Assists residents with planning and accessing recreational and social activities.

Poolside Aide

Part time position (Tuesdays, 12 – 4:30 p.m.) working at the pool at Eberhardt Center in Canandaigua during Aqua Therapy sessions. This person will be assisting clients and therapists throughout the sessions. Duties of the poolside aide include, but are not limited to, the following:

  1. Prepare pool area for treatment.
  2. Review therapist schedule for the day and pull charts.
  3. Take equipment out of locked closets.
  4. Client Care.
  5. Assist with dressing/redressing.
  6. Obtain equipment from deck area for therapist.
  7. Assist with transition to and from deck/pool.
  8. Supervise deck and pool related to safety issues.

Qualified Candidates Require:

  1. High School Diploma.
  2. Swim Instructor Certification preferred.
  3. Experience working with the public.
  4. One year experience working with individuals with disabilities.

Special Education Teacher

Part time vacancy (mornings) at the Geneva location working in an integrated preschool classroom as part of a comprehensive interdisciplinary team. Includes competitive compensation with comprehensive benefit plan. Special Education Teaching duties include but are not limited to:

  1. Administers standardized assessment tools to obtain data necessary for the development of the identified special needs student’s Individual Education Program (IEP).
  2. Develops the Individual Education Program (IEP) for each child identified utilizing parental involvement in the development of goals on the IEP.
  3. Organizes a structured instructional program providing for the individual developmental needs of special needs children in collaboration Teacher’s Assistant.
  4. Adapts and modifies the early childhood curriculum/lesson plans to meet the needs of the special needs children.
  5. Coordinates classroom team and planning meetings to facilitate appropriate programming for each child.
  6. Participates in school district Committee on Preschool Special Education and Committee on Special Education meetings.
  7. Orients, mentors and supervises new staff.
  8. Orients, mentors, supports and directly supervises 1:1 aides assigned to special needs children in their classroom.
  9. Supervise students, interns and volunteers placed in the classroom setting.
  10. Develops 1:1 Aide plans defining roles and responsibilities of 1:1 Aides assigned to special needs students in the classroom.

Qualified Candidates Require:

  1. Bachelor’s degree in Education with NYS Certification in Special Education.
  2. Experience with preschoolers or children with developmental delays.
  3. Knowledge of child development.

Respite Aides

Part time vacancies available in the Canandaigua area. Respite Aides provide in-home respite care (i.e. sitter service) to individuals as outlined by the Medicaid Service Coordination service plan and complete and submit monthly documentation representative of the service delivered. Respite Aides set their own schedules and work with families to set up visits at a mutually agreeable time. Rate is $11 per billable unit provided.

Qualified Candidates Require:

  1. High School Diploma
  2. Knowledge of community resources and means of accessing these services.
  3. Valid NYS driver’s license/dependable vehicle.
  4. Experience with special needs population.
  5. Demonstrates basic reasoning and writing skills.

Community Habilitation Specialist (Self-Directing)

Two vacancies working with an individual with a disability in the Canandaigua area. Weekly hours are 30 or 28. Includes competitive rate and comprehensive benefits. Duties include but are not limited to:

  1. Provides training to program participants to help them acquire daily living skills which will enable these individuals to function in more independent living situations.
  2. Provide functional assessment of a wide variety of life skills and provide appropriate feedback to program participants with regard to strengths, areas to develop and barriers to overcome.
  3. Use creative problem solving to help participants overcome barriers to independence.
  4. Develop appropriate service plan goals and activities to help consumers reach their goals.
  5. Advocate for person centered planning for program participants.
  6. Provides assistance and training to program participants to help them develop necessary problem solving skills which will enable them to better function in their community living situation.
  7. Accompanies program participants to locations in the community to help them learn how to access and utilize community resources, including health and safety resources.
  8. Assist program participants in accessing recreational and social activities, thus enabling them to utilize their leisure time appropriately.
  9. Acts as a role model for program participants by interacting with people in a manner, which will assist these individuals to develop behaviors necessary for successful community living.
  10. Assist with implementation of behavior modification plans as needed.

Qualified Candidates Require:

  1. High School Diploma or equivalent.
  2. Knowledge of community resources and means of accessing these resources.
  3. Valid NYS driver’s license.
  4. Demonstrates basic reasoning and writing skills.
  5. 6 months experience working with individuals with disabilities providing habilitation or rehabilitation and independent living skills training.

Full Day, Half Day and Substitute 1:1 Aides

Full day and half day vacancies at our Geneva and Waterloo site preschool programs. Substitute Aides needed for Canandaigua, Geneva and Waterloo preschool programs.

Typical duties of our Aides include but are not limited to:

  1. Implements educational, therapeutic and behavioral plans under the direct supervision of the special education teachers, therapy staff, psychologist and input from early education teacher for their assigned child.
  2. Assists the children throughout the day as directed by the Special Education Teacher.
  3. Supervises assigned child at all times (with the exception of lunch break, meetings and/or training), including during busing and transitional activities.
  4. Assists child in all activities of daily living while promoting independence.
  5. Functions as a positive, contributing member of the classroom team.

Qualified Candidates Require:

  • High School Diploma or equivalent
  • Experience with young children preferred.


Part time and substitute vacancies available for the Geneva and Canandaigua sites to assist with general cleaning duties. Qualified applicants require:

  • High School Diploma or equivalent.
  • 1-2 years of paid experience in general building maintenance and cleaning or demonstrated ability to successfully perform such duties.
  • Proven ability to work independently.
