Service Coordinator

Full time job opportunity at Canandaigua site providing information, referral and advocacy services to individuals of all ages with disabilities and their families. Works with families/individuals to develop appropriate plans for the delivery of services and coordinates implementation of the services by assuring appropriate communication and cooperation is maintained by all supports and providers of services outlined in the plan.

Qualified individuals include:

    1. Education and Experience Requirements:
    A. Masters Social Work, Masters Psychology, Registered Physical Therapist, Registered Nurse, Certified Special Education Teacher, Speech Language Pathologist, Registered Occupational Therapist with one year experience in providing service coordination/case management to individuals who have a disability; or
    B. Bachelor’s Degree in health and human services or a related field with two years experience in service coordination/case management; or
    C. Associate’s degree in human services field with three years experience in service coordination/case management.

2. Knowledge of community resources, social service agencies and means of accessing services.

3. Valid NYS driver’s license.

Community Habilitation Specialist

Full and part time opportunities in the Finger Lakes area working with individuals with disabilities in their home and community assisting them with their daily living skills which will enable these individuals to function in more independent living situations. Use creative problem solving to help participants overcome barriers to independence, develops appropriate service plan goals and activities to help consumers reach their goals, advocates for person centered planning for program participants, provides assistance and training to program participants to help them develop necessary problem solving skills which will enable them to better function in their community living situation.

Qualified applicants require a high school diploma or equivalent, knowledge of community resources and means of accessing these resources, valid NYS driver’s license, demonstration of basic reasoning and writing skills and experience working with individuals with disabilities providing habilitation or rehabilitation and independent living skills training.

Residential Assistant

Positions located in Gorham, NY in a residential setting for teens with disabilities. Successful applicants must have a high school diploma and valid NYS driver's license. Must be able to obtain and maintain Medical Certification. Experience with special needs population preferred. Substitute opportunities available. Full time evening shift position available as well as substitute opportunities.

Residential assistants provide oversight to ensure safety and supervision of residents. Provide assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to live more independently.

Accompanies residents to off-site locations/activities helping them to access and utilize community resources. Assists residents with planning and accessing recreational and social activities.

Personal Care Aide (PCA) / HCSSA

Full and part time opportunities at Canandaigua site working with adults with a Traumatic Brain Injury (TBI) in their home and community assisting them with daily living activities. Provides care in accordance with the Plan of Care and physician’s orders; after supervising RN has approved the individual to provide said care. The following cares may be provided by a Personal Care Aide (PCA):

    Assistance and training for daily living skills; dressing, bathing, feeding and grooming.
    Light cleaning, dusting, vacuuming and dishes.
    Shopping and running errands.
    Preparation of meals for regular and simple modified diets.
    Placement of and/or removal of elastic stockings, braces, splints, slings, prostheses.
    Obtain a weight.
    Assist with self-administration of oral medications (remind individual, read label, bring medication to the individual, open container, and/or provide a drink).
    Clean a stable wound with soap and water and apply a clean dry dressing.

Acts as a role model for program participants by interacting with people in a manner, which will assist these individuals in developing appropriate social behaviors necessary for successful community living. Applicants require a high school diploma, valid NYS driver’s license, knowledge of community resources and certification as a PCA in NYS. PCA certification may be obtained after beginning employment. Includes mileage reimbursement, competitive salary and comprehensive benefit plan.
