Service Coordinator Level 2

Full Time (37.5 hour) vacancy available at our Geneva location. Service Coordinators identify the needs of disabled individuals and work with the individuals/families to develop plans for the delivery of services. Service Coordinators duties include but are not limited to:

  1. Identifies the needs of disabled individuals and their families.
  2. Provides information, referral and advocacy services to individuals of all ages with disabilities and their families.
  3. Works with families/individuals to develop appropriate plans for the delivery of services.
  4. Coordinates implementation of the services by assuring appropriate communication and cooperation is maintained by all supports and providers of services outlined in the plan.
  5. Establishes and maintains a cooperative working relationship with area human service agencies, local school districts and individuals, as pertinent.
  6. Arranges for clinical assessments to determine the person’s functional status, service needs and recommendations for interventions.
  7. Assumes a lead role and coordinates all activities for people on caseload.
  8. Assists the consumer and/or family to gain a variety of skills including self-advocacy, problem solving and skills to increase their independence.
  9. Assures needed assessments and documentations are performed, as required by OPWDD regulations.

Qualified candidates require:

  1. Education Requirements:
    1. Bachelor’s degree in a health and human services field, or
    2. Degree in nursing as a Registered Nurse (RN)

    2. Bachelor’s Degree in health and human services or a related field with two years experience in service coordination/case management. Or
    3. Persons in category b with one year experience in service coordination who receive biweekly supervision from a person in category a.

Human Resource Generalist

Full time job opportunity working in the Human Resources Department at the Geneva site. The Human Resource Generalist is responsible for performing HR-related duties in supporting the HR Manager and Vice President and the overall HR Support to the Agencies. This position carries out responsibilities in the following areas: recruitment/employment, employment law compliance, onboarding and benefit administration. Position includes comprehensive benefit plan and competitive compensation. Duties include but are not limited to:

  1. Ensures compliance with all federal, state and local employment laws.
  2. Maintains all employee and applicant documentation as dictated by governing agencies.
  3. Implements full recruitment initiatives providing support to hiring managers to include but not limited to: internal and external job postings/ads, promoting the Agency’s job vacancies at job fairs, reviewing applications, screening applicants, performing reference checks, initiating offer letters, etc.

Quality Assurance Specialist and Structured Day Program (SDP) Case Manager

Full time opportunity working for Happiness House in the Structured Day Program as Case Manager as well as Quality Assurance Specialist supporting the QA Department for Happiness House and CP Rochester. Work sites would be Canandaigua and some travel to CP Rochester. Offers competitive salary with comprehensive benefit plan. Duties include but are not limited to:

  1. Completes thorough and objective investigations of incidents along with proposed plans of corrective action and maintains incident files.
  2. Completes routine internal audits of programs including chart, billing, and program audits.
  3. Fulfills the agency’s Incident Report and Management Application (IRMA) management responsibilities.
  4. Prepares policies for policy review committees.
  5. Ensures all internal and external incident reporting documentation and notification requirements are completed within their specified timeframes.
  6. Reports identified trends that impact the quality of services provided along with plans of corrective action to the Director of QA.
  7. Provide support to the External Corporate Compliance Committee, Human Rights Committee, Behavioral Rights Subcommittee and Agency Incident Review Committee.

Program Secretary (Canandaigua Preschool)

  1. Answers the telephone and greets all visitors to the program, directing them to appropriate locations.
  2. Types all program information for preschool and New Friends staff, including correspondence, supplemental evaluations, staffings, discharges, final progress reports and Individual Education Programs (IEP).

Special Education Teacher

Direct Support Professionals duties include but are not limited to:

  1. Provide oversight to ensure safety and supervision of residents.
  2. Provide assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to live more independently.
  3. Accompanies residents to off-site locations/activities helping them to access and utilize community resources.
  4. Assists residents with planning and accessing recreational and social activities.

Full Day, Half Day and Substitute 1:1 Aides

Full day and half day vacancies at our Geneva and Waterloo site preschool program. Substitute Aides needed for Canandaigua, Geneva and Waterloo preschool programs.

Typical duties of our Aides include but are not limited to:

  1. Implements educational, therapeutic and behavioral plans under the direct supervision of the special education teachers, therapy staff, psychologist and input from early education teacher for their assigned child.
  2. Assists the children throughout the day as directed by the Special Education Teacher.
  3. Supervises assigned child at all times (with the exception of lunch break, meetings and/or training), including during busing and transitional activities.

Maintenance Workers/Janitorial

  1. Sweep, mop, clean windows and vacuum floor surfaces and strip/wax floors according to cleaning schedule provided by Maintenance Supervisor.
