Employment Application

Registered Professional Nurse (RN)

Full time vacancy located at the Canandaigua site working Monday – Friday – 37.5 hours per week. Includes competitive salary with a comprehensive benefit plan. RN duties include but are not limited to:

  1. Works as the Article 36 Clinic Supervisor directing all home care services by providing assessment of clients, consulting with client’s primary physician and developing client care plans and determining service and supervising the implementation, coordination, maintenance and evaluation of care plans.
  2. Responsible for supervision of all services provided by Happiness House through the performance of on-site client visits, personnel evaluations, client satisfaction surveys, and other appropriate mechanisms.
  3. Promotes preventative and wellness health services to program participants and to employees as appropriate.
  4. Provides professional supervision to agency nurses to assure appropriateness of health and safety services.
  5. Ensures that agency medical practices and guidelines comply with all applicable regulations from DOH, OSHA, State Education Department and other regulatory agencies.
  6. Oversees Agency health and safety standards for all programs in conjunction with Program Directors.
  7. Will serve as the Agency health care consultant to oversee the daycare center’s health care plan under the Office of Children and Family Services (OCFS) regulations.

Qualified candidates require:

  1. Graduate of an accredited School of Nursing.
  2. Valid and current NYS Registered Nurse License and Registration.
  3. Minimum of three years nursing experience with one (1) year of home care experience in a LHCSA, CHHA, or LTHHCP preferred.
  4. One to two years of clinical experience, preferably working with adults with disabilities including those with traumatic brain injury.
  5. Minimum of one year management experience.
  6. Excellent verbal and written communication skills.
  7. Valid NYS driver’s license.

Service Coordinator

Full time vacancy at the Canandaigua location. Offers competitive salary with comprehensive benefit package. Service Coordinator duties include, but are not limited to:

  1. Provides information, referral and advocacy services to individuals of all ages with disabilities and their families.
  2. Works with families/individuals to develop appropriate plans for the delivery of services.
  3. Coordinates implementation of the services by assuring appropriate communication and cooperation is maintained by all supports and providers of services outlined in the plan.
  4. Establishes and maintains a cooperative working relationship with area human service agencies, local school districts and individuals, as pertinent.
  5. Arranges for clinical assessments to determine the person’s functional status, service needs and recommendations for interventions.
  6. Assumes a lead role and coordinates all activities for people on caseload.
  7. Assists the individual and/or family to gain a variety of skills including self-advocacy, problem solving and skills to increase their independence.

Qualified candidates require:

  1. Education and Experience Requirements:
    1. Masters Social Work, Masters Psychology, Registered Physical Therapist, Registered Nurse, Certified Special Education Teacher, Speech Language Pathologist, Registered Occupational Therapist with one year experience in providing service coordination/case management to individuals who have a disability. Or
    2. Bachelor’s Degree in health and human services or a related field with two years experience in service coordination/case management. Or
    3. Persons in category b with one year experience is service coordination who receive biweekly supervision from a person in category a.
  2. Knowledge of community resources, social service agencies and means of accessing services.
  3. Valid NYS driver’s license.

Direct Support Professional

Full time, part time, summer and relief positions located in Gorham, NY (between Geneva and Canandaigua) in a residential setting for teens and young adults with disabilities. Evening and overnight shifts available with competitive rates.

Direct Support Professionals duties include but are not limited to:

  1. Provide oversight to ensure safety and supervision of residents.
  2. Provide assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to live more independently.
  3. Accompanies residents to off-site locations/activities helping them to access and utilize community resources.
  4. Assists residents with planning and accessing recreational and social activities.

Qualified applicants require:

  1. High school diploma or equivalent
  2. Valid NYS driver's license.
  3. Must be able to obtain and maintain Medical Certification.
  4. Experience with special needs population preferred.

TBI Structured Day Program Assistant

Full time position working with individuals with Traumatic Brain Injury (TBI) in the Structured Day Program at the Canandaigua site, M-F. Position offers competitive rates and a comprehensive benefit plan.

Duties of the Program Assistant include, but are not limited to:

  1. Functions as the primary assistant to the TBI Program Specialist.
  2. Implements each program participant’s therapeutic and behavioral plans under the guidance of the TBI Program Specialist.
  3. Works with individuals on special instructional projects to achieve goals on each individual’s Program Plan.
  4. Provides the TBI Program Specialist with information about program participants, which will assist in the development of appropriate program experiences for all participants.
  5. Charts individual’s performance according to established guidelines and timeframes.
  6. Assists in the development of instructional materials.

Qualified Candidates Require:

  1. High School Diploma or equivalent.
  2. One year of experience providing services to individuals with disabilities, preferably working with individuals with traumatic brain injury.
  3. Knowledge of community resources and means of accessing these resources.
  4. Valid NYS driver’s license.
  5. Demonstrates basic reasoning and writing skills.

Special Education Teacher

Part time vacancy (mornings) at the Geneva location working in an integrated preschool classroom as part of a comprehensive interdisciplinary team. Includes competitive compensation with comprehensive benefit plan. Special Education Teaching duties include but are not limited to:

  1. Administers standardized assessment tools to obtain data necessary for the development of the identified special needs student’s Individual Education Program (IEP).
  2. Develops the Individual Education Program (IEP) for each child identified utilizing parental involvement in the development of goals on the IEP.
  3. Organizes a structured instructional program providing for the individual developmental needs of special needs children in collaboration Teacher’s Assistant.
  4. Adapts and modifies the early childhood curriculum/lesson plans to meet the needs of the special needs children.
  5. Coordinates classroom team and planning meetings to facilitate appropriate programming for each child.
  6. Participates in school district Committee on Preschool Special Education and Committee on Special Education meetings.
  7. Orients, mentors and supervises new staff.
  8. Orients, mentors, supports and directly supervises 1:1 aides assigned to special needs children in their classroom.
  9. Supervise students, interns and volunteers placed in the classroom setting.
  10. Develops 1:1 Aide plans defining roles and responsibilities of 1:1 Aides assigned to special needs students in the classroom.

Qualified Candidates Require:

  1. Bachelor’s degree in Education with NYS Certification in Special Education.
  2. Experience with preschoolers or children with developmental delays.
  3. Knowledge of child development.

Elementary Education Teachers

Full day and half day teaching opportunities to teach at the Midlakes UPK program located at the Phelps-Clifton Springs School District. Competitive salary with comprehensive benefit program. 10-month schedule beginning September 2017.

Elementary Education Teacher duties include but are not limited to:

  1. Completes all paperwork dealing with the referral and registration of new students.
  2. Administers developmental assessments to all students to gather data to determine how to meet individual development needs of the children.
  3. Coordinates daily and weekly lesson plans.
  4. Works with the Special Education Teacher to coordinate a structured program and safe environment that provides for the individual development needs of the children.
  5. Participates in all parent-teacher conferences for children in the New Friends program.
  6. Attends all team, discipline and staff meetings as designated.
  7. Supervises classroom aides and assistants, students and others involved in helping capacities with input from the Special Education Teacher.
  8. Completes annual performance appraisals for teaching assistants and aides with input from the Special Education Teacher agency policy.

Qualified candidates require:

  1. Bachelor’s degree in Education with NYS Certification in Elementary Education.
  2. Experience working with preschool children.
  3. Knowledge of child development.

Teacher Aides

Full day and half day (afternoon) positions working at the Midlakes UPK program located at the Phelps-Clifton Springs School District beginning September 2017 on a 10-month schedule. Competitive rates with benefits.

  1. Performs non-instructional duties under the supervision of the classroom teacher.
  2. Assists the classroom teacher in all children’s activities throughout the day.

Full Day, Half Day and Substitute 1:1 Aides

Full day and half day vacancies at our Geneva and Waterloo site preschool program. Substitute Aides needed for Canandaigua, Geneva and Waterloo preschool programs.

Typical duties of our Aides include but are not limited to:

  1. Implements educational, therapeutic and behavioral plans under the direct supervision of the special education teachers, therapy staff, psychologist and input from early education teacher for their assigned child.
  2. Assists the children throughout the day as directed by the Special Education Teacher.
  3. Supervises assigned child at all times (with the exception of lunch break, meetings and/or training), including during busing and transitional activities.
  4. Assists child in all activities of daily living while promoting independence.
  5. Functions as a positive, contributing member of the classroom team.

Qualified Candidates Require:

  • High School Diploma or equivalent
  • Experience with young children preferred.


Part time and substitute vacancies available for the Geneva and Canandaigua sites to assist with general cleaning duties. Qualified applicants require:

  • High School Diploma or equivalent.
  • 1-2 years of paid experience in general building maintenance and cleaning or demonstrated ability to successfully perform such duties.
